A Free Swinging KAFO is a type of Knee-Ankle-Foot Orthosis designed to provide stability to the knee in the medial-lateral (ML) plane—side-to-side stability— and against hyperextension without resistance during the swing or stance phases of gait.

Think of it as a very strong and supportive frame around your knee with a hinge; it allows free movement in one plane (front to back) while stabilising the knee in the side-to-side direction and against hyperextension. This type of KAFO is particularly suited for individuals who only require support to prevent the knee from hyperextending or moving side-to-side and those with lower limb weakness but good hip strength.

Who Would Benefit from a Free Swinging KAFO?

Patients who benefit from a Free Swinging KAFO often have neurological conditions such as polio or stroke, where the muscles controlling knee stability are compromised or injuries where integral structures are damaged.

Why?

Conditions can weaken the muscles and ligaments that stabilise the knee, leading to instability in the mediolateral (ML) plane. This instability can cause the knee to buckle inward or outward or hyperextend, increasing the risk of falls and damage to the knee structure. A free-swinging KAFO provides the needed support by limiting unwanted ML movement and hyperextension while allowing normal forward motion.

Free swinging KAFOs can also be used for those with weak knee extensors to allow them to lock the knee backwards for stability without causing damage over the longer term.

The joints are designed to facilitate movement in sagittal plane (front to back), preventing any unwanted side-to-side movement.

These devices are custom-fitted to the patient, with various options available depending on the required level of stability. Compared to stance control KAFOs, Free Swinging KAFOs are significantly lower cost.

However, the specific cost will depend on the components used and the complexity of the device. These costs are advised early in the process, so the client has choice and control.
